By accessing or using our website, booking services, or purchasing products from EnVogue Wellness & IV Solutions (“the Site”), you agree to comply with and be bound by the following Terms and Conditions. 1. General Information

EnVogue Wellness & IV Solutions is a nurse-led medical spa and wellness practice offering aesthetic treatments, IV therapy, and weight management programs, as well as a range of wellness and skincare products. All treatments are provided under the supervision of qualified medical professionals.

The information provided on this Site is for educational and informational purposes only and is not intended to replace professional medical advice, diagnosis, or treatment. Always consult your healthcare provider before starting any new treatment or program.

2. Eligibility

By using this Site or booking a service, you confirm that you:

  • Are at least 18 years of age, or have parental/guardian consent if under 18.
  • Provide accurate and truthful information during booking or checkout.
  • Will not use our services or Site for any unlawful or prohibited purpose.

3. Medical Disclaimer

All treatments and products offered by EnVogue Wellness & IV Solutions have been deemed safe based on current clinical standards but have not been evaluated or approved by the U.S. Food and Drug Administration (FDA). They are not intended to diagnose, treat, cure, or prevent any disease.

Individual results may vary, and no guarantees are made regarding treatment outcomes. The results of any wellness, weight loss, or aesthetic treatment depend on individual factors, including age, metabolism, health status, and adherence to post-treatment recommendations.

Good Faith Examination Requirement

Prior to receiving any medical or aesthetic service, all patients must undergo a Good Faith Examination (GFE) conducted by a licensed medical provider. This ensures the patient is medically eligible for treatment and that all services are safe and appropriate.

  • The Good Faith Exam is valid for one (1) year from the date of approval, unless a change in health status occurs that requires re-evaluation.
  • All treatments must be pre-approved by a licensed provider before being performed.

If you have a chronic or high-risk medical condition such as uncontrolled diabetes, heart disease, or other ongoing health concerns, please consult with your primary care physician before receiving any services.

We do not provide treatment to patients who are:

  • Currently diagnosed with or undergoing treatment for cancer
  • Living with kidney disease
  • Pregnant or nursing
  • Experiencing active skin infections, open wounds, or inflammatory skin flare-ups (such as eczema or psoriasis)

Please inform our staff if you are under medical treatment, taking prescription medications, have new or ongoing health changes, or have metal implants/devices (such as pacemakers, plates, or screws), as these may affect treatment safety or eligibility.

By scheduling or receiving services, you acknowledge that you have read, understood, and agree to comply with this medical disclaimer and all eligibility requirements.

4. Appointments and Scheduling

All services require an appointment. In some cases, a consultation may also be required prior to treatment. Appointments can be scheduled online, by phone, or in person.

Late arrivals (more than 10 minutes) may result in shortened treatment time or rescheduling. We reserve the right to cancel or reschedule appointments due to unforeseen circumstances or provider availability.

5. Payments and Pricing

Payment is required at the time of booking or service. We accept major credit cards, FSA/HSA, CareCredit, and approved payment plans.

Prices for services and products are subject to change without notice. Promotional offers cannot be combined unless explicitly stated.

All deposits and prepayments are non-refundable but may be applied as credit according to our Refund Policy.

6. Refund and Credit Policy

All sales are final. We do not offer refunds on services, packages, or products after purchase.

If a patient is determined medically ineligible for a treatment by our medical provider, a refund will be issued for that specific service.
For all other cases, no refunds will be provided. A credit may be issued toward another service of equal or lesser value at management’s discretion.

Credits must be used within 90 days of the original purchase unless otherwise stated.

7. Cancellation and No-Show Policy

To cancel or reschedule your appointment, please contact us at least 24 hours prior to your scheduled time.

Late cancellations or no-shows may result in the loss of your deposit or a fee equivalent to the scheduled service.
Repeated no-shows may result in suspension of booking privileges.

8. Product Sales and Shipping

All product purchases (including skincare, supplements, and digital items) are final sale and non-refundable.

We make every effort to process and ship orders promptly; however, shipping times may vary based on location, inventory, and carrier performance.
EnVogue Wellness & IV Solutions is not responsible for lost, stolen, or delayed packages once shipped.

9. Memberships and Packages

Memberships, bundles, or multi-treatment packages are non-transferable and non-refundable.
Missed or unused sessions cannot be rolled over beyond the stated term unless otherwise approved in writing.

10. Limitation of Liability

By using our Site or receiving our services, you agree that EnVogue Wellness & IV Solutions, its owners, employees, contractors, and affiliates shall not be liable for any damages, losses, or injuries resulting from your use of our products or services.

To the maximum extent permitted by law, we disclaim all warranties, express or implied, and limit our liability to the amount paid for the service or product in question.
